If you have a different private insurance plan, we are happy to provide a super bill upon request. You can then submit it to your insurance provider for possible out-of-network reimbursement. Self-pay rates are also offered if you choose not to submit to insurance. Payment is due at the time of service, and we accept various forms of payment for your convenience.
